What Makes TIL Therapy Different

Unlike drugs designed for a category of patients, TIL therapy starts with cells extracted directly from your specific tumor. Those cells already recognize your cancer โ€” the therapy dramatically changes the numbers.

โ€œThe intelligence is already there. TIL therapy just changes the numbers.โ€
  • Deeply Personalized

TIL cells are extracted from inside your specific tumor โ€” not a generic drug designed for a category of patients. The immune memory is already built against your cancer.

  • Lab-Amplified Response

From a starting population of thousands, scientists grow your TIL cells into tens of billions โ€” then return them to your body in a state where they can actually fight back.

TIL Therapy at a Glance

  • 2024FDA Approval YearLifileucel approved for advanced melanoma โ€” the first TIL therapy approved globally.
  • ~31%Response Rate (Melanoma)Objective response in heavily pre-treated patients who had failed prior immunotherapy.
  • 4โ€“6 wksCell Expansion PeriodTime required in a GMP-certified lab to grow TIL cells from thousands to billions.

How TIL Therapy Works โ€” A Quick Overview

TIL therapy moves through five key stages. Each step sets up the next โ€” the sequence cannot be skipped.

  1. 1

    Tumor Tissue Collection

A surgeon removes a small piece of the tumor โ€” the source of your immune cells.

  2. 2

    Cell Extraction & Expansion

Lab scientists isolate TIL cells and grow them from thousands to tens of billions over several weeks.

  3. 3

    Lymphodepletion Chemotherapy

A short conditioning regimen clears immune space so the new TIL cells can establish and function effectively.

  4. 4

    TIL Cell Infusion

Expanded cells are infused intravenously โ€” similar in appearance to a blood transfusion.

  5. 5

    IL-2 Support & Recovery

Interleukin-2 is given to sustain the infused cells. Response is assessed by imaging at 6โ€“8 weeks post-infusion.

Where TIL Therapy Stands Today

TIL therapy has been in development since the 1980s. In February 2024, the FDA approved lifileucel (Amtagvi) for unresectable or metastatic melanoma โ€” the first TIL product approved anywhere in the world. Trials in lung, cervical, head and neck, and other solid tumors are actively enrolling.

TIL Therapy vs. Standard Immunotherapy

Patients who have already tried checkpoint inhibitors often wonder if TIL therapy is simply more of the same. It is not.

TIL Therapy

  • Made from your tumorCells extracted specifically from your cancer tissue.
  • Active after immunotherapy failureWorks through different biology โ€” resistance to PD-1 drugs does not predict TIL failure.
  • Multi-target recognitionTIL cells recognize multiple tumor antigens, not a single engineered target.

Checkpoint Immunotherapy

  • Same drug across all patientsNot personalized to individual tumor biology.
  • Releases an immune brakeBlocks PD-1 or CTLA-4 signals to restore existing immune response.
  • Outpatient every few weeksLess intensive delivery, but limited options once resistance develops.
