What Conventional Immunotherapy Does

Checkpoint inhibitors โ€” pembrolizumab, nivolumab, atezolizumab โ€” block proteins like PD-1 or CTLA-4 that cancer cells use to send a "stand down" signal to the immune system. They do not add new immune cells; they remove a brake, allowing the immune system already present to respond more aggressively.

Where TIL Therapy Comes In

TIL therapy does not release brakes. It introduces billions of tumor-educated immune cells that operate independently of the pathways targeted by checkpoint inhibitors. The resistance mechanism that overcame a PD-1 drug is not the same mechanism that TIL cells rely on โ€” which is exactly why prior immunotherapy failure does not predict TIL therapy failure. The melanoma clinical data demonstrated this directly: a meaningful proportion of patients who progressed on anti-PD-1 therapy still had measurable responses to TIL therapy.
