The treatment of cancer is being revolutionized by new immunotherapeutic drugs that target the microenvironment at the tumour site, with CAR-T cell therapy at the forefront of this transformation. Tisagenlecleucel, a CD19-specific CAR-T cell, has received clinical approval, and CAR designs targeted at novel targets involved in haematological and solid malignancies are being tested in ongoing clinical trials. The landscape of CAR-T cell therapy in China has grown at a very rapid pace, with recruitment for clinical trials ongoing at several leading cancer centers including Tianjin Hematology Hospital, Anhui Provincial Hospital, Peking University Hospital Shenzhen, the 1st Affiliated Hospital of Soochow University, and the 3rd Xiangya Hospital of Central South University. The study employs a 3+3 dose exploration design for JWCAR029, a CD19-targeted CAR-T cell therapy, evaluating four dose levels during the treatment period, with dose-limiting toxicity assessed within 28 days after infusion and at least one pediatric subject younger than 10 years enrolled at each dose level.

Eligible patients for the JWCAR029 clinical trial must be aged 30 years or younger with a weight of at least 10 kg, and must have relapsed or refractory B-ALL with morphological disease in the bone marrow showing at least 5% blasts. Patients must also demonstrate adequate organ function, a Karnofsky or Lansky performance status above 60, and an expected survival time of more than three months, while females of childbearing potential must agree to use highly effective contraception for one year following infusion. On the other hand, patients are excluded if they have active central nervous system leukemia with significant neurodegenerative symptoms, existing clinically significant CNS conditions such as epilepsy or stroke, active HBV, HCV, HIV or syphilis infection, uncontrolled systemic infections, or a history of cardiovascular diseases such as Class III or IV heart failure within the past six months. Those who have previously received CAR-T cell therapy, other gene-modified T cell therapy, or any anti-CD19 therapy are also ineligible for participation in the trial.
