HER2-Targeted Drug Options
Most patients receive more than one HER2-directed agent across different phases of their treatment. The available options continue to expand.
Trastuzumab (Herceptin)
Foundational HER2 antibody still used across early-stage and metastatic disease, typically in combination with chemotherapy or other targeted agents. Biosimilar versions widely available globally.
Pertuzumab
Binds to a different HER2 site than trastuzumab. The HP doublet is standard in early-stage HER2+ disease, particularly before surgery (neoadjuvant). Together they provide more complete HER2 blockade than either alone.
T-DXd (Trastuzumab Deruxtecan / Enhertu)
Next-generation ADC that has reshaped metastatic HER2+ treatment. Also active in HER2-low breast cancer -- a category that previously had no HER2-targeted options. Phase III ORR above 60% in heavily pretreated patients.
Tucatinib
Oral TKI targeting HER2 from inside the cell. Shows particular activity in patients with brain metastases. Combined with trastuzumab and capecitabine in the HER2CLIMB trial.
T-DM1 (Kadcyla)
ADC linking trastuzumab to a cytotoxic agent. Used in patients with residual disease after neoadjuvant therapy or after first-line metastatic treatment.
