How NGS Findings Connect to Treatment
Two routes exist for the connection between a finding and a treatment recommendation. The clean route and the less clean route.
Companion Diagnostic -- The Clean Route
FDA-approved companion diagnostic identifies the alteration. An approved drug is indicated for that alteration in that cancer type. Osimertinib for EGFR mutations in NSCLC. Alpelisib for PIK3CA in HR+/HER2- breast cancer. The treatment follows directly from the result.
Tumour-Agnostic Approvals
Applicable regardless of cancer type. NTRK fusions: larotrectinib or entrectinib. MSI-H: pembrolizumab. TMB-high: pembrolizumab. RET fusions: selpercatinib. These exist specifically so that cancer type does not block access to a matched treatment when the molecular target is present.
Molecular Tumour Board Review
For complex NGS cases where findings do not map to established indications, multidisciplinary review by oncologists, molecular pathologists, and bioinformaticians can identify potential strategies. Available at many academic centres and through virtual consultation services.
Basket Trial Enrollment
For alterations without approved matched therapy, a biomarker-selected basket trial may be the most relevant access point. For specific alterations in cancer types without targeted therapy approvals, it is sometimes the most clinically justified option currently available.
