LYMPHODEPLETION BEFORE
TIL THERAPY
Chemotherapy before an immune therapy β it seems contradictory. Here is why lymphodepletion is the step that makes everything else work.
What Lymphodepletion Actually Does
Lymphodepletion temporarily reduces the existing immune cell population in the body. Without clearing that space first, billions of TIL cells struggle to survive, expand, and function. Lymphodepletion creates a temporary window where new TIL cells can land, establish themselves, and start doing their job.
βThink of lymphodepletion as clearing a space before filling it with something that needs room to grow.β
Medications Used in Lymphodepletion
Most TIL programs use a combination of two agents, given intravenously over the conditioning period.
| Drug | Class | Role |
|---|---|---|
| Cyclophosphamide | Alkylating agent | Depletes lymphocytes β not aimed at tumor cells in this context |
| Fludarabine | Purine analogue | Immunosuppressive agent with strong effectiveness at reducing lymphocyte populations |
What Patients Actually Experience
Significant Fatigue
Expected and universal. Rest is necessary throughout β the immune system is under intentional stress.
White Blood Cell Drop
Temporary but real. This is the most significant risk window β prophylactic antibiotics and antifungals are given as standard.
Why Timing Between Lymphodepletion and Infusion Is Critical
TIL cells are infused within 24 hours of completing the conditioning regimen β not logistical convenience, but biological necessity. The window created by lymphodepletion is time-sensitive. As soon as conditioning ends, the body begins repopulating immune space. This is one reason the entire TIL process requires tight coordination between the lab, pharmacy, and clinical team.

How CancerFax Helps
CancerFax is a specialist cancer access and patient-navigation platform. We help patients and families understand their options, organise medical records, coordinate hospital communication, and support cross-border treatment planning where appropriate.
We help collect and organise reports, scans, pathology, biomarker results, and treatment history for structured case review.
We communicate with hospitals or trial teams to assess whether a case may be suitable for further screening.
We support appointment coordination, document submission, translation, and direct communication with international departments.
For international patients, we help with practical coordination β travel planning, hospital admission guidance, and local support.
If this option is not suitable, we help explore other relevant treatments, clinical trials, or advanced care pathways.
From inquiry through to follow-up, our coordinators provide a single point of contact for the family.
CancerFax does not guarantee treatment access, eligibility, or clinical outcome. Our role is to help patients access accurate information, structured review, and appropriate specialist pathways.
