Why the Immune System Is Suppressed After CAR-T
CD19-targeting CAR-T eliminates all CD19-positive cells β including normal B cells, creating B-cell aplasia and inability to produce new antibodies.
B-Cell Aplasia Duration
B-cell aplasia typically lasts 3β12 months after CD19 CAR-T. In some patients it persists longer, particularly with deep ongoing CAR-T persistence. Recovery confirmed by B-cell count returning to normal.
Hypogammaglobulinaemia
Without B cells, IgG falls progressively over months. Low IgG increases risk of bacterial infections. IVIG replacement every 3β4 weeks is the main intervention when IgG < 400β500 mg/dL.
Immune Recovery Timeline

Weeks 1β4: Acute Phase
CRS and ICANS management. Neutropenia from lymphodepletion. Prophylactic antibiotics, antifungals, and antivirals initiated. High infection risk β hospitalisation standard.

Months 1β6: B-Cell Aplasia
B cells absent or very low. IgG falling. IVIG every 3β4 weeks if IgG < 400 mg/dL. PCP, antiviral, and antifungal prophylaxis continued. No live vaccines.

Months 6β12: Early Recovery
B cells beginning to return. IgG may still be low β continue IVIG as needed. CD4 T cells recovering. Gradually reduce prophylaxis as counts normalise.

12+ Months: Late Recovery
Most patients have restored B cells by 12β18 months. Live vaccines after 24 months if immune competence confirmed. Ongoing monitoring for late infections.

About Post-CAR-T Immune Recovery
How long should IVIG continue after CAR-T?
IVIG is continued as long as IgG is below threshold (usually 400β500 mg/dL) or until B cells recover. Many patients require it for 6β18 months. Monthly or 3β4 weekly infusions are standard, adjusted based on IgG trough levels.
When can vaccinations be given after CAR-T?
Live attenuated vaccines are contraindicated until immune competence is confirmed β at least 24 months post-infusion and after B-cell recovery. Inactivated vaccines can be given after B-cell recovery but may have reduced immunogenicity. Your CAR-T centre will provide a personalised schedule.
