The Full Arc of CAR-T Treatment

From decision to infusion, most patients face 4-8 weeks. The timeline is built into the biology โ€” cells need to be collected, modified, expanded, and quality-tested. Understanding each phase helps patients prepare for a significant but structured commitment.

8 Phases of CAR-T Treatment

  1. 1

    Pre-Treatment Evaluation & Consent

    Disease confirmation, organ function, cardiac baseline, infectious screening, and substantive consent conversation.

  2. 2

    Leukapheresis

    T-cells collected via apheresis. Day procedure, 3-6 hours. Mild fatigue common.

  3. 3

    Manufacturing

    T-cells genetically modified with CAR construct and expanded. Patient at home or receiving bridging therapy.

  4. 4

    Bridging Therapy

    Treatment to keep disease stable during manufacturing. Not always needed; type depends on cancer and urgency.

  5. 5

    Lymphodepletion Chemotherapy

    Fludarabine + cyclophosphamide for 3 days. Creates immune space for CAR-T engraftment.

  6. 6

    CAR-T Infusion

    IV administration, less than an hour. The complexity is in the days that follow.

  7. 7

    Post-Infusion Monitoring

    Inpatient 7-14 days. CRS and ICANS monitoring with vital signs, neuro checks, cytokine levels.

  8. 8

    Follow-Up & Response Assessment

    Regular blood tests, imaging. Response evaluated at ~1 month, then at intervals.
